Dynamic stabilization/soft stabilization uses flexible materials to stabilize the affected lumbar region while preserving the natural anatomy of the spine. It is intended to alter the load-bearing pattern of the spine’s motion segment and to control any abnormal motion while leaving the affected spinal segment mobile. The idea behind stabilization is that controlling abnormal motions while supporting more physiologic load transmission may relieve the pain associated with degenerative disk disease and prevent further degeneration of adjacent segments of the spine. Once normal motion and load transmission can be achieved, a patient’s damaged disc may even repair itself.

How is the Lumbar Dynamic Stabilization Surgery performed?

The goal of lumbar dynamic stabilization/soft stabilization is preservation of as much of the natural spinal anatomy as possible, while maintaining flexibility and allowing the different segments of the spine to move and rotate normally. Depending upon the nature of the disease affecting the lumbar dynamic stabilization patient, the surgeon may perform decompression of nerve roots, reformation of vertebral facets and repair/removal of affected disk segments as necessary to augment the stabilization procedure, though each dynamic stabilization procedure has the following in common:

Surgery is performed under sedation plus local anesthesia in a hospital operating room, with almost all procedures taking a posterior approach to accessing the spine. Unlike the implantation of rigid rods used with spinal fusion surgery, lumbar dynamic stabilization surgery uses lumbar pedicle screws, which are inserted into the strongest part of the vertebrae above and below the source of the patient’s low back pain. An active rod that acts like a suspension spring is then placed between the screws to provide the stabilization needed, while preserving spinal movement.

A feature of lumbar dynamic stabilization that makes the procedure more attractive than traditional spinal fusion is that it is less invasive. The surgical technique required for lumbar dynamic stabilization/flexible stabilization involves making a smaller incision, reducing the amount of damage to the tissues beneath the skin.

Less invasive spine surgery uses specially designed instruments, which aid the surgeon’s visualization of the area to be operated on, typically employing tubular retractors which allow the surgeon to make a small incision but visualize a larger area beneath this small incision.

Less invasive surgical techniques are in continuous development and are currently used to treat conditions such as spondylolisthesis and degenerative disc disease, two conditions for which lumbar dynamic stabilization has shown promise as an effective treatment.

The benefits of less invasive spine surgery may include reduced scaring following the operation and less damage to the surrounding tissues beneath the skin, including less pain and blood loss. The benefits may also include a quicker postoperative recovery and a quicker return to a normal lifestyle.

Are you an ideal Candidate for Lumbar Dynamic Stabilization Surgery?

There are currently several indications for patients who might benefit from lumbar dynamic stabilization.

In the US, all the lumbar dynamic stabilization systems/devices thus far approved by the FDA have been certified for use in conjunction with spinal fusion. When used with fusion, the goal is to adequately brace the spine so fusion occurs but without the excessive rigidity conveyed by metal rods. There is some evidence that interbody grafts fuse at a higher rate when they are compressed, which occurs better with dynamic stabilization than with traditional rigid instrumented fusion.

Lumbar dynamic stabilization/flexible stabilization can also be used to recreate the posterior tension band. The ‘posterior tension band’ is part of the spine’s anatomy found at the back of the spine (posterior). Elements that make up the posterior tension band are the spinous processes and lamina (bony plate that is part of each vertebral body) along with the ligaments that join these. In many cases, these posterior elements are removed during a spinal decompression procedure, which can potentially destabilize the spine. Dynamic stabilization/soft stabilization restores the posterior tension band (posterior support) without the need for spinal fusion.

Building a transition zone to bridge a fused segment to a non-fused segment by spreading the forces across multiple spinal levels is another use for which the flexible devices used in lumbar dynamic stabilization have shown promise. The analogy here is akin to the plastic or rubber collar on an electrical cord that spreads forces between the highly flexible cable and rigid plug, giving greater integrity to the whole system.
