Patients considering LASIK to address vision problems will first have their eyes examined to determine their suitability for the procedure and to see if there is any disease or other condition present, that could negate the advisability of having the surgery. The doctor will use the exam to determine what kind of vision correction is needed, and how much laser ablation is required to achieve the desired result. A corneal topographer may be used to photograph the eye and create a detailed “map” to guide the surgeon in making the proper adjustments during the procedure.

People both nearsighted and farsighted can benefit from LASIK. Nearsightedness is corrected by flattening a too-steep cornea; with farsighted people, a steeper cornea is desired. While it is not widely recognized by many, Excimer lasers can also correct astigmatism by smoothing an irregular cornea into a more normal shape.

The standard LASIK procedure is ambulatory; patients walk into the surgery center, have the procedure and walk out again. In fact, the actual surgery usually takes no more than five minutes, and the patient remains awake the whole time. In some cases, the surgeon may administer an oral sedative to help patients relax before the procedure.

After anesthetizing the eyes with special drops, the surgeon will place the patient face-up directly beneath the laser, with a kind of retainer used to keep the eyelid open during the procedure. Normally, this is not uncomfortable. The retainer has a suction ring that keeps the eye pressurized, which is important in LASIK to allow the surgeon to cut the corneal flap.

The surgeon will then use an ink marker to mark the cornea before the flap is created. The flap is then created with either a microkeratome or with a laser, depending on the surgeon’s preference.

Using a computer to adjust the laser for the patient’s prescription, the surgeon will have the patient look at a target light for a short time while the doctor watches the eye through a microscope and the laser sends pulses of light to the cornea.

Laser light pulses then painlessly reshape the cornea. Patients will hear a steady clicking sound as the laser is operating and may smell a mildly acrid odor due to the removal of tissue. The surgeon has full control of the laser and can turn it off at any time. Once the adjustments to the cornea are complete, the surgeon replaces the previously cut flap, which acts as a kind of natural bandage and is self-healing.

In most cases, if both eyes are being done, the surgeon will ask the patient to rest for a short time between sessions with the laser. Some patients choose to wait a week or so between sessions.

LASIK eye surgery is an excellent option for patients who wish to decrease or eliminate their dependence on glasses or contact lenses. However, to be considered appropriate candidates for LASIK, patients’ existing vision defects must fall within an acceptable LASIK correction range and must not have changed considerably for at least one year prior to surgery.

The amount of refractive error that a patient can overcome with LASIK depends on many factors, including the skill of the surgeon, the parameters of the chosen Excimer laser and the central thickness of the patient’s cornea.

Currently accepted correction ranges for standard LASIK treatment of refractive defects are as follows:

  • Myopia: the standard range for LASIK extends from -0.75 to -10.0 D (diopters) of shortsightedness, with an extended range up to -12.0 D of shortsightedness.
  • Hyperopia: LASIK has been FDA approved for correction of farsightedness up to +6 D. However, LASIK results are more predictable for corrections up to + 4 D.
  • Astigmatism: between +/- 0.75 to +/- 4.00 D can be corrected by LASIK.

While LASIK has been used with some success to correct refractive errors beyond these ranges, many surgeons limit LASIK correction to levels lower than those used a few years ago. Years of clinical data support the conclusion that more precise results are obtained from LASIK surgery in lower ranges than in higher ranges of correction.

In addition, alternatives to LASIK now exist for people with higher degrees of refractive defects. With new ranges of approval for higher corrections in Wavefront-guided LASIK, patients needing higher levels of correction may now benefit from improvements like those enjoyed by patients with lower levels of myopic and hyperopic corrections over the past several years.

In addition to the corrective parameters available to prospective LASIK candidates, patients should also meet the following general criteria:

  • Patients should be 18 years or older and in good general health
  • Patients’ eyes should be healthy i.e., no dry eye disease, infection or glaucoma
  • Eye refraction should be stable for at least one year
  • Patients should not be taking any prescribed medication such as oral prednisone
  • Patients should be neither pregnant nor nursing
  • Patients should be suffering from no autoimmune or degenerative diseases.

Benefits of LASIK Surgery

Patients with poor natural vision who need eyeglasses or contact lenses to function in their daily lives may contemplate many enjoyable benefits from LASIK surgery. The procedure itself takes very little time and requires no general anesthesia or overnight hospital stay; recovery is fast, with many patients able to see just as well or better than they did with glasses or contacts mere hours after surgery.

LASIK patients no longer need glasses or contacts to drive (or pass driving tests) and can play sports and enjoy other outdoor activities that were once difficult to participate in.

In addition to the convenience of hassle-free eyesight, LASIK eye surgery patients also find career opportunities open to them that are off-limits to those with poor vision, such as working as a military or commercial airline pilot. Military service personnel are also able to extend their careers beyond normal timeframes after being approved for LASIK eye surgery.

Beyond the joy and freedom of attaining clear vision without glasses or contact lenses, LASIK patients often feel a sense of improved self-esteem and almost all LASIK patients report improved post-surgical quality of life.

Possible Complications

In general, when performed by a competent, experienced surgeon who has a clear understanding of the patient’s goals and expectations, LASIK is one of the safest, most effective and predictable types of surgery one can have today. As with any surgery, however, LASIK carries certain risks that every patient should understand and discuss completely with their surgeon prior to committing to the procedure.
